Inclusive Horizons is a nonprofit organization providing individualized community integration services and behavioral supports for adults with disabilities in San Benito County. We support individuals with intensive needs through intentional, person-centered programming that prioritizes dignity, safety, and meaningful participation in the community.
Our services are designed to promote independence, skill development, and social connection through structured supports, therapeutic approaches, and real-world community experiences. We work closely with each individual and their support team to create personalized plans that reflect their strengths, goals, and support needs.
Inclusive Horizons was created for individuals who have not thrived in traditional program models. Our approach is flexible, relationship-based, and highly individualized, allowing each person to engage in their community in ways that are purposeful, empowering, and sustainable.
Inclusive Horizons provides weekday community-based services for adults ages 18 and older with disabilities, including but not limited to autism spectrum disorders, intellectual disabilities, and developmental delays. Our services are designed to support individuals with behavioral and intensive support needs through intentional structure, evidence-informed interventions, and individualized support plans.
Programming is delivered through a combination of structured activities, skill-building opportunities, and supported community engagement. Each individual’s day is thoughtfully planned to promote safety, regulation, independence, and meaningful participation based on their unique strengths and goals.
